Delivering care

Improving Chronic Illness Care (ICIC) launched as a national program of the Robert Wood Johnson Foundation in 1999. For almost a decade, ICIC has worked with national partners with the goal of bettering the health of chronically ill patients by helping health systems, especially those that serve low-income populations, improve their care.

Pyramid has supported the communications needs of the program since its inception, including branding, messaging, materials and website development, special events and knowledge sharing. To address the numerous and diverse requests for update-to-date, evidence-based information about their Chronic Care Model and its application in a variety of healthcare settings, we worked with the ICIC team to create a series of interactive CDs.

Each CD directs viewers to current information on the ICIC website, allows advocates of the model to easily and flexibly create customized presentations, and lets users share inspiring video content with their audiences. The series has become an anchor in the dissemination of the Chronic Care Model.
